Growth opportunities We are currently seeking an experienced and compassionate Home Health Licensed Practical Nurse to join a prominent home health agency in Zaneville, OH.
As a Home Health Licensed Practical Nurse (LPN) you will be responsible for providing exceptional care to patients in their homes.
If you have a minimum of 1 year of recent Home Health experience and are dedicated to delivering compassionate care, we encourage you to apply for this lucrative position.
Responsibilities:
Provide high-quality and compassionate nursing care to patients in their homes Conduct comprehensive assessments of patients' health status and develop individualized care plans Administer medications and treatments as prescribed by physicians Monitor and document patients' conditions and progress Educate patients and their families on managing their health conditions and promoting wellness Collaborate with interdisciplinary healthcare team members to ensure coordinated care delivery Maintain accurate and up-to-date medical records Benefits Offered by Ascendo Healthcare Staffing:

Active Ohio Licensed Practical Nurse License (Required) Minimum of 1
